FAMILY CARE COORDINATOR
12 Month Contract
Start Date: April 22, 2013
The Family Care Coordinator works from within an
interdisciplinary team model to support the assessment
and treatment needs of young people experiencing a first
episode of psychosis and their families. The Family Care
Coordinator is able to integrate medical, clinical and
rehabilitation approaches to facilitate and maintain
meaningful recovery from psychosis. Responsibilities
include: processing referrals for service, family work,
outreach and engagement; and group facilitation.
1. Degree in Social Work (masters preferred), or
Psychology or Registered Practical Nurse with mental
health training. A combination of education and
experience may be considered.
2. Minimum 3-5 years clinical experience in a mental
health setting, preferably delivering counseling
services to youth and families
3. Registration with the relevant professional college
4. Demonstrated knowledge of psychotic illness, from
both a medical and psychiatric rehabilitation approach
5. Demonstrated knowledge and skills in completing
comprehensive individual and family assessments as
part of intake planning and ongoing treatment
6. Excellent oral and written communication skills
7. Experience delivering health teaching in a group or
individual format
8. Willingness to travel within the region
9. Demonstrated ability to work effectively and
collaboratively within an interdisciplinary team setting
